Event: 10/06/1993

Employee injured in fall through roof opening

At about 7:28 a.m. on October 6, 1993, Employee #1, a welder, fell over 20 feet to a dirt floor from the roof of a warehouse under construction. He was working near a 6 foot by 6 foot unguarded and uncovered skylight opening, and while pulling on a welding cord he fell backwards. Employee #1 fell head first but held onto the welding cord, causing him to rotate and strike the ground with his feet or back. No one witnessed the accident, but they heard the noise of his fall and came to his aid. 911 was called and paramedics arrived in about 5 minutes. The unconscious employee was flown to a local hospital emergency room. Employee #1 suffered a lower back fracture and two fractures above his right elbow.
