ELECTRIC SHOCK - DIRECT CONTACT WITH ENERGIZED PARTS

An employee was troubleshooting an electric disconnect switch located on top of a water tank. He did not deenergize the circuit before beginning this work. The employee apparently contacted an energized part and was electrocuted.
