Investigation Summary
At approximately 10:13 a.m. on October 18, 1993, Employee #1 was on top of a 20 foot extension ladder painting the outside stucco wall when he fell and landed on his arms and right hip. The ladder was placed on a makeshift table (sawhorses with a large plank on top) to obtain additional height. The ladder was not tied or secured. The sawhorses tipped outward causing the ladder's feet to slide away from under the employee. Employee #1 fell face down, holding the ladder, and rolled on his right. Employee #1 suffered fractures to both hands and the right ribs. He was taken to the hospital for treatment. The employer had been painting the large two story house since July 1993 and hired Employee #1, a day worker, to help for a few days. There were no witnesses to the accident. According to the employer, Employee #1 was a trained house painter with 15 years experience. The employer told Employee #1 to paint the whole wall but then left to make a phone call. Employee #1 was on his third ladder move when he fell. The employer was found in violation of 8ccr1675(j), and a citation was issued as serious with penalty.
