Investigation Summary

Investigation Nr: 170625677
Event: 09/12/2001
Employee Killed When Struck By A Train

On September 12, 2001, Employee #1 unloaded bulk transport railroad cars of cattle feed. The rail cars were parked next to an active railroad track. The cattle feed was stuck together from the heating and cooling of the material. The employees hit, vibrate, and poke the material to loosen it up, so it will flow out the bottom of the rail car. Employee #1 was attaching a 3-foot model longslide air vibrator to the under side of the hopper. He finished attaching the air vibrator and came out from under the rail car on the active track side. A scheduled train was passing by and struck Employee #1's head, killing him.
