Employee Suffers Crushed Foot

Employee #1 was parking her stand-up forklift next to the warehouse office. Employee #1 was getting close to a parked electric pallet jack. Employee #1's left foot was out of the operator compartment. Employee #1 tried to make the forklift go forward, but inadvertently made it go in reverse. Employee #1's foot was caught between her forklift and the forks of the parked pallet jack causing injury to her left foot. Employee #1 was hospitalized.
