Employee Is Crushed by Hydraulic Cylinder

On April 16, 2004, Employee #1 was assisting two other employees repairing a CNC vertical mill. Employee #1 was working at the rear of the machine. The other employees were ready to start the machine, and the employee at the controls yelled to Employee #1 to clear the machine. He thought Employee #1 was clear and started the machine. Employee #1 was pinned against the wall of the machine by a large hydraulic cylinder. Employee #1 sustained crushing injuries to his pelvis, liver, kidney, and back. He was admitted to UMC Hospital in Fresno, CA.
