Employee injures foot when struck by forklift

On March 23, 2001, Employee #1 was operating a hand rider pallet jack. The employee had just finished talking to a female employee and started driving to the staging dock with a load of product, still looking at the female employee. Employee #1 drove approximately 40 to 50 ft to go down the ramp. At the same time, there was another forklift coming up the ramp. The other driver saw Employee #1 coming toward him and started honking the horn to get his attention. Employee #1 did not respond, so the other forklift driver started to back up, but one of the forks hit Employee#1's left foot, lacerating it. The employee was transported to the hospital and treated for his injuries.
