Investigation Summary
On March 13, 2003, Employee #1 was assigned to start and retrieve a fork lift from the yard. The fork lift had been sitting for a long time. He started it up and discovered that the throttle was sticking out about 0.5 to 0.75 inches. The employee drove the fork lift to the shop to have the throttle repaired. When he showed the service manager the problem, he reached in and started the fork lift. The fork lift moved forward quickly, and the employee hit the floor board to stop it. The fork lift hit a post and pinned Employee #1 between the post and the beam. The service manager used another fork lift to free the employee. Employee #1 sustained a serious contusion on his right leg, and was taken to UMC, where he was treated and released.
