Investigation Summary
On February 10, 2003, Employee #1 was assigned to do clean-up on the process piping. He started to work as instructed. He was told to cut off the hot water supply to the Rodger's dryer. The ball valve was worn and did not close all the way. He apparently did not know that fact at that time. He continued his duties. He unhooked the elbow on the flow vent panel and the pipe was filled with hot water. This water was pressurized and sprayed Employee #1, burning the front of his body. He was hospitalized.
