Investigation Summary
Employee #1 was assigned to cut fiber optic cable on the wire and cable cutting machine. The fiber optic cable was hard to cut with the machine because it was filled with jelly. This jelly got on the floor and made it slippery. Employee #1 was working in the jelly and slipped. He tried to catch himself by grabbing the machine. The machine was cycling at the time and caught the tip of his finger. He was taken to the hospital to treat the amputation of his finger.
