Investigation Summary
Employee #1 was cleaning a peeler machine, which was left running to reach all parts of the rollers. He was working when his left glove became caught in the rollers and pulled in his hand. Employee #1's index finger was amputated, his ring finger was partially amputated, and he sustained multiple lacerations to his other fingers.
