Employee's fingers amputated

The employee was assigned to form a wire cage for a concrete structure. He fed the re- enforcement mesh wire into the roll form machine. He was holding the end of the mesh and it caught his glove. He tried to pull his hand loose but it went into the roller. The employee's left hand fingers were amputated. He was hospitalized for his injuries.
