Investigation Summary
Employee #1 was assigned to cover bins of raisins temporarily due to weather conditions. He was walking across a stack of bins approximately 18 ft tall. There was a cavity in the stack near the center of the structure. He was pulling the black plastic across the bins while walking on top. He was advised of the cavity. Employee #1 was busy pulling the black plastic and did not pay attention and stepped into the cavity. He fell down and eventually fell on his head injuring it. He was taken to the hospital for treatment.
