Employee Injured When Run Over By Forklift

On December 15, 1999, Employee #1 was assisting the forklift operator load bulk sand bags by holding the lifting straps up, so the operator could insert the tines through the loops and lift the bags. After the bags were lifted, they were taken from the warehouse to a staging area outside for subsequent shipping. The forklift operator had taken approximately six bags outside and was returning to the warehouse to get another one. After entering the warehouse and traveling 35 ft, the operator made a right turn. The forklift operator said that he blew the horn when he entered the warehouse however Employee #1 said he did not hear the horn. When the operator made this turn, he was not aware that Employee #1 had walked from the location where he loaded the last bag to the east side of the building to get his coffee that was on a bag. Employee #1 was struck by the forklift and sustained a compound fracture to his right leg and a crushed left foot. Employee #1 was hospitalized.
