Employee killed when run over by grader

At approximately 8:30 a.m. on October 1, 1993, Employee #1 was assigned to help a grader operator backfill an area next to a newly constructed roadway. Employee #1 was watching the end of the grader blade to ensure that it did not damage the new asphalt or other objects that had been installed at the roadside. This assignment, called a "guinea hopper," requires the employee to walk near the end of the blade with a shovel and fill in depressions, and to guide the operator who is driving the grader. There was other equipment working in the area at the same time. A "sidewinder" and hauling trucks were on the same side of the road going in the opposite direction putting dirt on the berm next to the curb. The grader operator decided to turn around and work the area behind the sidewinder. The grader operator stopped to check traffic to the rear of the grader before turning into the roadway, and signaled for the guinea hopper to get on the equipment. The grader operator then looked to his left to check traffic, looked to his right, saw Employee #1 climbing onto the grader, looked back to his left to check for traffic, and then started out. Employee #1 was run over and killed by the grader's tandem wheels.
