Investigation Summary

Investigation Nr: 170605927
Event: 08/28/1993
Employee injured when struck by piece of rebar

At approximately 7:22 p.m. on August 28, 1993, Employee #1 was working at a rebar building station. He was marking a spot on the ground with his foot and leaning with his right hand on a rebar previously positioned in the bundling station cradle. Due to Employee #1's bent-over position, the pulpit operator did not see him in the danger zone and advanced the conveyor system. A piece of #18 rebar fell onto Employee #1's right hand, resulting in injuries for which he was hospitalized.
