Employee injured in fall from elevated, unsecured platform

At approximately 9:45 a.m. on June 23, 1993, Employee #1 was standing on an unsecured platform elevated by a forklift. He was installing steel storage racks inside a large warehouse under construction. A coworker tossed an object to Employee #1. When Employee #1 leaned outside of the cardboard box he was standing in, he and the makeshift platform fell a distance of 12 feet, resulting in injuries for which he was hospitalized. The employer was cited for a serious violation of T-8ccr 3657 a 2 and 3.
