Employee is crushed and killed by pipe

At 8:00 a.m. on December 21, 2016, an employee in the grinding area was operating a hammer mill to reduce off-spec PVC pipes (16 in. diameter, 20 ft. long, 1,000 lb. approximate weight each). The employee was pushed into the machine chute by several pipes. The pipes then fell into the chute and crushed the worker, who was killed.
