EMPLOYEE FINGERS AMPUTATED BY PANEL SAW

At 7:00 a.m. on March 8, 1994, Employee #1 reached into the point of operation on a panel saw and two fingers of his right hand were partially amputated. Employee #1 was the panel saw operator and removed stock panels from the table of the panel saw after the cutting operation was completed. Four wood panels were placed on the table by a second operator who controlled the operation of the table saw. Employee #1 reached out to remove cut stock or unintentionally reached into the unguarded point of operation and was seriously injured. The adjustable guard had not been properly adjusted and then secured in place to guard Employee #1 from contacting the saw blade at the point of operation. Employee #1 was hospitalized for approximately 8 hours. There was a difference of observation between the accident witness and Employee #1.
