Investigation Summary
At approximately 1:00 p.m. on September 16, 1993, Employee #1 was operating an electric pallet jack as part of his training during his second day on the job. He made a pick-up, left the pallet and cartons to be wrapped, and returned to the rack area to do more work. His immediate supervisor directed him to pick up a pallet in aisle 18. As Employee #1 proceeded down the aisle, he lost control of the pallet jack and knew that it was going to collide with the rack. He put his left foot out to keep the pallet jack from hitting the rack and became caught between the pallet jack and the rack. Employee #1 was hospitalized for more than 2 months for the injuries he sustained.
