Investigation Summary

At approximately 9:00 a.m. on March 15, 1994, Employee #1, of ESD Company, was at a construction site as a skip loader was unloading conduit from his stakebed truck. Employee #1 was standing on the truck bed. As the forks of the skip loader approached to pick up a section of conduit, one of the forks struck a piece of wood (dunnage). Employee #1 stepped with one foot over the pipe onto the edge of the truck bed. His foot slipped and he fell off the truck to the ground. Employee #1 was hospitalized as a result of his injuries.
