Investigation Summary
At approximately 3:45 p.m. on March 31, 1993, Employee #1 was wrapping plastic around a flat order in aisle 22, near rack 22-73-OA, when he stepped over the connecting bar between two flats and into the aisle as a forklift drove by. Employee #1's right foot was run over by one of the forklift's trailing wheels, causing serious injuries for which he was hospitalized. The forklift operator had sounded his horn, thought he made eye contact with Employee #1, and had removed his foot from the accelerator.
