Investigation Summary
At approximately 1:30 p.m. on September 24, 1992, Employee #1, of Inland Empire Equipment, was repairing and replacing the lifting chain on a Princeton D4500 forklift. The length of chain used to immobilize the forklift mast broke, causing the forklift mast assembly to lower abruptly and amputate Employee #1's right arm.
