Employee killed in fall from mezzanine

At approximately 1:15 p.m. on September 6, 1992, Employee #1, of C.T. Williams Construction, was working on a 40 ft by 24 ft mezzanine addition located 16 ft above the original floor of the building. Employee #1 had been instructed to remove a pair of boots lying in the area where concrete was to be poured. The decking steel and reinforcement wire were in place and concrete had been poured on part of the mezzanine floor. The pump being used to transport concrete to the second floor had to be replaced because of mechanical problems. The employer turned his back to direct another employee, and when he turned back he found Employee #1 had fallen off the side to the floor. He was killed by the fall. There were no guard rails installed on the perimeter of the mezzanine.
