Investigation Summary
Employee #1 was reinforcing a railroad under-crossing and preparing forms to pour concrete. Employees had stood the two side walls of the poured-in-place structure. Employee #1 was working on the top of a 10 ft high 2 by 4 form. He had just placed a 4 by 4 joist across the two forms to place the roof on the form and was moving to the next joist by placing one foot on the joist he had just set in place and the other foot on the 2 by 4 form wall. As Employee #1 stepped onto the 4 by 4, it broke and he dropped 10 ft to the concrete floor. Employee #1 sustained head injuries that required 10 days of hospitalization and at least six months leave from work. Employee #1 was not using fall protection or a work platform.
