Investigation Summary
Employee #1, a carpenter, was on a roof 9 1/2 ft above a concrete slab removing a catwalk. The roof had just been sheeted with 5/8 in. wafer board. The 4 ft by 8 ft wafer board failed at a seam over a 6 ft by 6 ft skylight opening where two full sheets met. Employee #1 fell and suffered a fractured hip and bruised ribs and wrist. He is undergoing physical therapy and will not return to work in the foreseeable future.
