Employee injured when struck by falling bundle of plywood

At approximately 3:30 p.m. on July 24, 1997, Employee #1, a truck driver, was standing beside a truck when a forklift operator swerved to avoid hitting him. A bundle of 4 by 8 sheets of plywood slipped and fell off the side of the forklift's tines, striking Employee #1 in the back and landing on top of him. He sustained crushing injuries and was transported by helicopter to Loma Linda University Medical Center for treatment.
