Employee's fingers amputated in stock cutter

Employee #1 was feeding rubber stock material through a stock cutter that had been adjusted to cut 1 in. square rubber pieces for molding into circular door stops via an injection molding process. When the strips of rubber became stuck on the cutting knife, Employee #1 placed his left hand under the grated guard on the discharge side of the machine to dislodge them. The machine was in operation and he amputated one or more of his fingers. Employee #1 would not provide a signed statement to collaborate the circumstances surrounding his injury.
