Employee's leg injured when struck by forklift tine

At approximately 8:20 a.m. on May 24, 1996, Employee #1 and a coworker were stocking products in a cooler of the receiving department of the Price-Costco Depot #957/972 warehouse facility. Employee #1 was operating Crown forklift 15A, serial #1A147258, and his coworker was using Crown forklift 3B, serial #1A136749. They were moving in the aisle between cooler storage rows #4 and #5 when the lift tines of the coworker's forklift entered Employee #1's compartment. He sustained a lacerated right thigh and fractured femur. The forklifts were not being operated correctly or with the drivers under full control. The employer was cited for a serious violation of T8CCR 3664(a).
