Investigation Summary
At approximately 3:45 p.m., on October 11, 1999, employee for JS Fabrication, Inc., Employee #1 sustained amputation of his left index finger and laceration of middle finger while he was removing a 8 in. diameter grooved steel pipe from a hydraulic "VICTAULIC" grooving machine, Model VE-260. Employee #1's finger got caught between the pipe and a lower rotating flange when the machine activated inadvertently with foot pedal. The machine was located in the SW part of the Fabrication area. Employee #1 was taken to Arrowhead Regional Medical center, Colton, CA. for treatment.
