Investigation Summary
At approximately 1:00 p.m. on April 17, 2001, Employee #1 was pushing boards on the mechanical rollers which pushed the boards on the belt conveyor. The belt conveyor fed the boards into the rollers of a "MONCO" laminating machine, Model #TB60, Serial Number 150088. During the laminating process, a coworker, the machine operator, noticed some glue stains on the rolls and he decided to wipe clean the rollers with WD-40 and a rag. The coworker asked Employee #1 at the far end of the operation (35-40 feet away) to not push boards onto the mechanical rollers. As the machine operator was cleaning the slowly moving rollers from the out-feed side, he noticed the head of Employee #1 behind the rollers. The coworker immediately pushed the emergency stop button at which time Employee #1's right hand sustained crushing injury in the rollers. Employee #1 came on his own to clean the rolls without the knowledge of the machine operator. This was not Employee #1's assigned duty. Employee #1 was hospitalized.
