Employee's fingers amputated on table saw

At approximately 3:30 p.m. on June 28, 2000, Employee #1, of Precision Designs, was using a tilting Arbor 3,450 rpm table saw, model JT AS-10-03, serial #706404, to cut a 4 in. thick by 14 in. diameter ring from a 36 in. long tube of 3/16 in. thick plywood. The shape and size of the part being cut required that he remove the guard from the blade. He was cutting the part when his left hand slipped and his middle, ring, and little fingers struck the blade and were amputated. Paramedics transported Employee #1 to Chino Valley Hospital, where he was retained for over 24 hours. The employer was cited for a serious violation of T8CCR 4296(a).
