Investigation Summary
At approximately 1:35 p.m. on September 7, 2000, Employee #1 was walking along side a Gradeall forklift operated by a coworker. The operator attempted to avoid another forklift when he turned the forklift slightly. The rear wheel of the forklift ran over Employee #1's right foot that caused him to be hospitalized for two days.
