Investigation Summary
At approximately 4:00 p.m. on September 7, 2000, Employee #1 had finished using a Clark 25 E forklift to transport pallets of fertilizer throughout the field. Upon completing the job, he backed the forklift onto a trailer via a ramp. The forklift tipped over and fell onto Employee #1. He was hospitalized for one month with multiple injuries.
