Investigation Summary

Investigation Nr: 201156361
Event: 07/16/2002
Forklift Runs over Employee's Foot

At approximately 9:30 a.m. on July 16, 2002, Employee #1 was leaving the shipping and receiving office walking in the pedestrian walkway, with his left foot three feet outside of the pedestrian walkway, when a Crown Stand-Up Forklift started to approach. The operator of the Crown Stand-Up Forklift was transporting a pallet of Kibble and Bits dog food from the special project area to the staging area and had to pass the shipping and receiving area where Employee #1 was standing. The operator saw Employee #1 standing in the pedestrian walkway and continued to move forward. As the operator passed Employee #1 with the front of the forklift, he noticed that Employee #1 had gotten closer. The right back wheel of the Crown Stand-Up Forklift ran over Employee #1's left foot. Employee #1 was hospitalized for 5 days and treated for his injuries.
