Investigation Summary
At approximately 11:15 a.m. on May 9, 2000, Employee #1, of Gene Blek Fruit Packers, was using an Urschell Labs dicing machine, serial #246, to process bell peppers. The dicing machine was jamming every 5 to 10 minutes. After some time, Employee #1 stopped the machine and removed the cover guard from the blade, manually cleaning out the material jamming the blade. He then turned the machine back on, but left the blade cover off. His right middle and ring fingers became caught by the blade and were amputated. Employee #1 was transported by paramedics to Arrowhead Regional Hospital by paramedics, where he was treated and released after approximately six hours. The employer was cited for a serious violation of T8CCR 3314(a) and T8CCR 3942(a).
