Investigation Summary
At 3:10 p.m. on February 11, 1991, Employee #1 was performing maintenance work with a coworker on a blender-outfeed conveyor. He entered the conveyor to remove/replace rakes when the conveyor began to operate. Employee #1 was pulled under the conveyor by the rakes. He died while being transported to the hospital.
