Investigation Summary
At approximately 7:40 a.m. on December 22, 1999, Employee #1, a parts washer operator, employed by ATC Distribution Group, suffered second and third degree burns to his lower legs and feet. Employee #1 stepped onto a solution tank cover that was not secured and fell into a 27-inch-deep parts washer solution tank that contained water and a cleaning compound heated to 170 degrees Fahrenheit. Coworkers helped him out of the tank. He was taken to the Arrowhead Regional Medical Center where he was hospitalized for 24 days.
