Employee's fingers injured in steel form roller

At approximately 2:26 p.m. on February 3, 2000, Employee #1, a steel form roller operator, was working at a manufacturing facility. His hand became caught between two pieces of rebar that he was processing and the rollers of the steel forming machine. He sustained crushing and amputation injuries to his left thumb and index and middle fingers. Employee #1 was escorted to Arrowhead Regional Medical Center, where he was hospitalized for six days.
