Investigation Summary
On Aug 9, 1999, Employee #1 was cleaning up around the oversized potato washer and thought he could remove some weeds in the lower conveyor area of the machine. He reached in underneath the conveyor and his hand was caught in the chain and sprocket drive which amputated four fingers and the tip of the thumb of his right hand. He was hospitalized.
