Investigation Summary
On June 8, 1999, a serious accident occurred at the EDM Training Center of the Department of Water and Power in Sylmar, California. An apprentice lineman, who was in training, was climbing poles at the training facility. He did a "free climb" to the top of the pole, approximately 25 ft. and was getting ready to descend when he "cut out". His boot gaffs came out of the pole and he began falling. He attempted to cut back into the pole, but the rate of descent when he cut back in caused his hands to break free from the pole and he fell backwards off of the pole landing on his neck and shoulders. He was taken to Holy Cross Presbyterian Hospital for emergency treatment.
