Employees toes are amputated when run over by forklift

Employee #1, working with two others, was assisting in the movement of a stone slab, weighing approximately 600 pounds and measuring approximately 5 ft by 9 ft. It was being moved from a storage area to a truck. Employee #1 was in front of the forklift to the right side of the stone slab. Another worker was located in front of the forklift to the left side of the stone slab, and the third worker was operating the forklift. As the forklift operator drove forward, the other two were walking along holding the stone to steady it. Employee #1 indicated that the forklift hit a bump or an uneven surface in the yard and the stone slab swung forward away from him. He grabbed the stone slab with both hands, trying to steady it. It swung back toward him and the force pushed him backward towards the forklift and the wheel ran over his left foot. Employee #1 could not see the driver of the forklift and the forklift operator could not see Employee #1 due to the slab. Employee #1's left foot was crushed and resulted in amputation of all five toes. He was hospitalized.
