Investigation Summary
On April 26, 1999, Employee #1 stated that he and two coworkers had traveled to a roofing supply company in a 20-ft panel dump truck to pick up supplies. After arriving at the supply company, he climbed into the bed of the truck and started arranging materials. His supervisor went inside the company while a coworker stood next to the truck waiting for the materials to be brought to the truck. When the supplies arrived, the employee on the ground started to hand them up to Employee #1 in the truck. After they had loaded all the materials, Employee #1 started to climb out of the truck. He placed one leg over the side panels and stepped onto the edge of the truck bed. He then placed his other leg over the side panels. With both hands on top of the panels and his feet on the edge of the truck bed, he jumped off the truck. When he jumped, the ring on his middle finger of his left hand caught on the edge of one of the steel side-support rails of the truck, causing the flesh to tear off his finger. His coworker, who was walking back toward the truck, helped him into the office and paramedics were called. Employee #1 was taken to the hospital and hospitalized. His middle finger was amputated and his index finger was replaced in the middle finger position. Employee #1 was released from the hospital the next day.
