On March 12, 1999, Employee #1, a machine operator, fell through an unguarded deck-mounted trapdoor floor opening. The accident was unwitnessed, and Employee #1 stated that he was moving some hoses, but does not remember falling or tripping. He was hospitalized for his injuries.
