EMPLOYEE #1 REACHED UNDER THE GUARD OPENING, WHICH WAS 3 INCHES BY 6 INCHES AT THE FEED END, AND GOT HIS HAND CAUGHT IN THE ROTATING SCREW. HE LOST SEVERAL FINGERS AND PART OF HIS HAND.
