Employee injured in fall through roof opening

At approximately 1:15 p.m. on November 17, 1992, Employee #1, of Hawker Pacific, Inc., attempted to help two coworkers who were working on the roof removing and installing corrugated metal roofing. The two coworkers had removed a piece approximately 5 ft by 3 ft from around a water cooler, and were behind the cooler (about 6 ft high) cutting another piece and removing metal supports from the water cooler. They were unaware that Employee #1 had come up on the roof until they heard him say, "Do you need help?" One coworker yelled, "Look out for the hole," but Employee #1 had reached the opening, stepped through the insulation, and fell approximately 18 ft to the concrete floor. Employee #1 sustained a fracture to the bone under one eye, two broken wrists, and a broken kneecap. He was hospitalized for surgery from November 17 to November 25, 1992.
