Investigation Summary
Employee #1, of Viking Equipment Corp., was cutting off a length of 10-in. I-beam while standing on a scaffold platform 6 ft above the floor. When Employee #1 cut through the beam, it fell, striking the platform of the scaffold and knocking him off. Employee #1 sustained a fractured left rib and a small laceration under his left arm.
