Investigation Summary
On August 3, 2012, employees of United Bark Products LLC where working on a belt conveyor, pulling off sticks, when Employee #1 caught his hand in the belt which pulled his hand under the head pulley. The belt conveyor head was not guarded, which caused his hand to be caught in the nip point between the conveyor and the head pulley. Employee #1 was taken to the hospital for his injuries which consisted of right hand nerve damage and dead tissue. Employee #1 was hospitalized for four days, then again for fifteen days, then an additional two days.
