Employee Is Injured When Tailgate Sliped

On March 8, 2012, Employee #1 was putting the tailgate back on a dump wagon. While the employee was holding the control lock with his right hand, the tailgate lock slipped smashing the employee's index finger of his left hand. Employee #1 was taken to an area hospital, where his index finger was amputated to the second knuckle.
