Investigation Summary
On June 4, 2020, employees #1 and #2 of Siemans Water Tech Corp., were performing routine maintenance on a series of interconnected augers. Employee #1 locked out the #2 auger and Employees #1 and #2 removed it from the casing which opened a discharge chute exposing the #3 auger which was not locked out. Employee #2 attempted to clear debris from the chute and got his left glove caught by the auger. Employee #2's left index finger was amputated and the left middle finger was severely fractured.
